Would a t18 to dana 20 adapter work for adapting a t98a to a large bore dana 18? Or is the rear of the 2 transmissions different?

I think it will work as long as the 18 is a large hole

I thought the bearings were different? That's why I am thinking it would not work?

There is no difference between the back side of the T98 and the back side of the T18.

The only thing different about CJ with T98 and CJ with T18 is the TC adapter.
    T98 CJ's were always mated to a small hole D18 @ 3-5/32" index.
    T18 CJ's were always mated to a large hole D18 or D20 @ 4" index.

Basically they use two distinct yet nearly identicle adapters.
The 3-3/32" adapter says "T98" on it.
The early mfg. 4" index adapters also say "T98".
    I think the later 4" adapters say "T18" ???

If you need to, you can get the adaptor milled to fit a small hole D18 - did that with a setup for the kids.
