    What would cause my transmission not being able to go In 1st or reverse when truck is running but will go right in gear when transfer case is in neutral ?

    I'd say clutch adjustment since 1st and reverse aren't synchronized.

    Clutch dragging, as Glenn suggested, you should check the linkage and action.
